NURSING ASSISTANCE IN THE MANAGEMENT OF ORAL MUCOSITIS IN ONCOLOGICAL PATIENTS

Oral mucositis is a sequel of citoredutivo treatment induced by radiotherapy and / or chemotherapy, with signs and symptoms ranging from pain, swelling and burning sensation, to ulcerations in the oral mucosa. The nursig staff shoud be able to offer therapies that give the patient better quality of life, once because the professionals devolps closer contact with the patient and family. To analyze the scientific evidences about nursing assistence in the management of oral mucositis in oncology patientes. Integrative Review in databases LILACS, Scopus, SciELO, LILACS and BDENF with complete articles published in the last 10 years, in Portuguese, English and Spanish. It was demonstrated that nurses can provide treatments such as oral hygiene, administration of analgesics, cryotherapy, treatment of xerostomia, chlorhexidine rinses, sodium bicarbonate and other substances that promotes pain control in patients with consequent increase of quality of life. The impact of nurses' actions in improving the patient, control of oral mucositis, as well as studies that generate strong evidence for the development and / or enhancement of new nursing care are aspects to consider this issue.

NURSING ASSISTANCE FOR CHAGAS DISEASE

Chagas Disease is an anthropozoonosis, caused by a trypanosomatida, of the Trypanosoma cruzi species, characterized by being an endemic, parasitic and biphasic clinical course disease, which may be manifested in undetermined, cardiac, digestive or cardiodigestive forms. To present a literature review about Chagas Disease proposing reflection on nursing attention in this context. This is a literature review, developed from articles indexed in BDENF, MEDLINE, and LILACS databases. Chronic patient has loss in the quality of life. Vector transmission is related to chronic cases. Oral transmission is the most worrying in the North of the country, being açaí the main food. Nursing care should improve the patient’s quality of life. It is suggested to invest in awareness and training strategies for nursing professionals aiming to instrumentalize them for a better assistence.

HOSPITAL ACCREDITATION: A NEW DIRECTION FOR NURSING

The increasing world competitiveness has been raising the necessity of evaluating the quality of health care organizations. Due to that, the hospital accreditation process has been born. To spread knowledge about hospital accreditation to students and health professionals. The chosen kind of research was bibliographic, based on literature review of scientific articles, and descriptive, as there have been observation, registration and analysis without affecting the subject. The results have evidenced the trajectory in the search for accredited hospital certification and the inclusion of methods and processes related to providers of health care accredditation services. In order to get hospital accreditation, initiatives must be taken so as to increase assistance standards. Thus, there must be comportamental changes and professional mobilization to achieve proposed goals and purposes, besides permanent and continuous improvement of the provided attendance. Moreover, that would garantee patient safety and the improvement of the quality of the provided services.

Nursing Strategies for the Control of Systemic Erythematosus Lupus

Systemic Lupus Erythematosus is a chronic condition, of autoimmune, inflammatory and multisystemic origin, with a higher incidence in women in the reproductive period. This condition can spread abd affect other systems, including psychosomatic dimensions. It can be triggered by environmental, immunological, genetic, and external factors such as drugs. Based on the clinical aspects found in literatures of the classic form of the disease, it is proposed to draw up nursing strategies for coping with it. The coping and self-preservation strategies were: Guidance on the exposure and use of sunscreens, demonstration of well-being positions to ease discomfort (gastric and respiratory), stimulate physical exercise, establish routine nursing for evaluation of blood count and vital signs, refer the patient to the nutrition service whenever necessary, and offer psychological support for the acceptance of his or her own image. In the presence of clinical findings (tissue damage resulting from photosensitivity, cutaneous and articular lesions and oral ulcers, etc.), laboratory tests (hemogram, FAN (antenatal factor), among others) and clinical manifestations presented by patients with the disease (fever, loss of weight, renal problems, pleuritis, myalgias, pericarditis, gastrointestinal problems, peripheral neuropathies and low self-esteem), there is a great advance in the clinical and psychological improvement of the patients who use these therapeutic techniques, as a consequence of the diagnosis and early interventions. These interventions promise to trigger a self-esteem improvement in the patients, as well as to broaden the discourse.

OS CUIDADOS DE ENFERMAGEM AOS PORTADORES DE HANSENÍASE

A hanseníase é uma doença infectocontagiosa causada pelo Mycobacterium leprae e é considerada uma das mais antigas doenças descritas na literatura. A bactéria causadora tem afinidade pelas células cutâneas e pelos nervos periféricos. Clinicamente caracteriza-se pela presença de manchas dormentes esbranquiçadas ou avermelhadas localizadas principalmente nas regiões de face, braços, pernas e costas. Os pacientes podem ainda apresentar lesões neurológicas expressas por dor e diminuição da força motora. Quando não tratada precocemente, a Hanseníase pode evoluir para incapacidades e deformidades físicas ocasionadas pelo agravamento das manifestações clínicas como, garra de artelho, lagoftalmo e absorções ósseas. Nessa perspectiva, ser portador de Hanseníase repercute significativamente no psicológico além de trazer restrições à vida social. Sendo assim, devido ao intenso impacto causado pela doença, é primordial o tratamento e controle da transmissão da doença. Destacar a importância do cuidado da enfermagem aos portadores de Hanseníase. Realizou-se uma revisão integrativa dos periódicos publicados nas bases de dados SCIELO, LILACS, BDENF e na Biblioteca Virtual de Saúde (BVS), utilizando os descritores “Cuidado”, “Enfermagem” e “Hanseníase” nos idiomas português, inglês e espanhol. Os critérios de inclusão foram artigos completos publicados em português, inglês ou espanhol, que abordassem o tema escolhido, publicados nos últimos 10 anos. Foram localizados 35 artigos dos quais 12 estavam de acordo com a temática e os critérios prédeterminados, portanto esses foram lidos integralmente. Os estudos elucidaram que, antes mesmo do diagnóstico definitivo, a hanseníase gera perturbações ocasionadas pelo conhecimento histórico das manifestações clínicas. A patologia desencadeia no indivíduo problemas psicossociais tendo em vista a estigmatização da doença evidenciado pelas alterações corporais. O portador de Hanseníase não é só acometido pelo bacilo, mas também se encontra em estado de total vulnerabilidade psicológica ao ponto de se isolar socialmente. A repercussão negativa da doença contribui para a evolução da mesma, em conjunto com a consolidação das limitações físicas e da queda na qualidade de vida. Nessa acepção, é necessário o cuidado do enfermeiro buscando amenizar o impacto emocional ocasionado pela doença, entendendo os aspectos psicológicos e planejando a assistência do cuidado em virtude da melhora do quadro clínico. O Brasil é líder mundial em prevalência da hanseníase e aproximadamente 40 mil novos casos são diagnosticados por ano. Destacamos o papel do profissional de enfermagem na adesão ao tratamento e orientações quanto as formas de contaminação. Assim, além de apoiar os portadores de Hanseníase no enfrentamento da doença, fortificando o autocuidado para prevenir as incapacidades e as restrições sociais os enfermeiros devem reforçar atenção para colaboração no tratamento, melhoria da qualidade de vida e redução do número de novos casos de hanseníase no Brasil.

NURSING CARE TO HANSENIASIS CARRIERS

Leprosy is an infectious disease caused by Mycobacterium leprae and is considered one of the oldest diseases described in the literature. The causative bacteria have affinity for cutaneous cells and peripheral nerves. Clinically it is characterized by the presence of whitish or reddish dormant spots located mainly in the face, arms, legs and back regions. Patients may also present neurological lesions expressed by pain and decreased motor power. If not treated early, leprosy can progress to physical disabilities and deformities caused by worsening of clinical manifestations such as, toe claw, lagophthalmos and bone absorptions. In this perspective, leprosy has significant repercussions on the psychological as well as bringing restrictions to social life. Therefore, due to the intense impact caused by the disease, the treatment and control of the transmission of the disease is very importance. To emphasize the importance of nursing care to patients with leprosy, an integrative review of the journals published in the databases SCIELO, LILACS, BDENF and the Virtual Health Library (VHL) was carried out using the descriptors "Care", "Nursing" and "Leprosy" in Portuguese, English and Spanish. The inclusion criteria were complete articles published in Portuguese, English or Spanish, which approached the chosen topic, published in the last 10 years. A total of 35 articles were found, of which 12 were in agreement with the thematic and the pre-determined criteria, therefore they were read integrally. The studies elucidated that, even before the definitive diagnosis, leprosy generates disturbances caused by the historical knowledge of the clinical manifestations. The pathology triggers in the individual psychosocial problems in view of the stigmatization of the disease evidenced by the body changes. The leprosy patient is not only affected by the bacillus, but is also in a state of total psychological vulnerability to the point of social isolation. The negative repercussion of the disease contributes to its evolution, together with the consolidation of physical limitations and the decline in the quality of life. It is necessary to seek to alleviate the emotional impact caused by the disease, understand the psychological aspects and plan care assistance due to the improvement of the clinical picture. Brazil is the world leader in leprosy prevalence and approximately 40,000 new cases are diagnosed per year. We highlight the role of the nursing professional in adherence to treatment and guidelines on the forms of contamination. Thus, in addition to suppor patients with leprosy in coping with the disease and fortifying self-care to prevent disabilities and social restrictions, nurses should reinforce attention to collaboration in treatment, improvement of quality of life and reduction of new cases of leprosy in the Brazil.

DESVENDANDO A HANSENÍASE: UMA ABORDAGEM PARA A ASSISTÊNCIA DE ENFERMAGEM

A hanseníase é uma doença infectocontagiosa muito antiga que representa, ainda hoje, um grave problema de saúde pública. Causada pelo Mycobacterium leprae, uma bactéria intracelular obrigatória que tem tropismo pela pele e nervos periféricos. No Brasil a hanseníase é diagnosticada tardiamente, aproximadamente, um ano e meio após o aparecimento dos primeiros sintomas. A falta de informação e a busca tardia pelos serviços de saúde contribuem para o diagnóstico tardio da doença. A Estratégia de Saúde da Família como porta de entrada para os serviços de saúde é responsável pelo diagnóstico, pelo tratamento e por ações educativas para os acometidos com a enfermidade, bem como para seus familiares e toda a comunidade. O profissional enfermeiro como parte integrante da equipe de saúde da família desenvolve atividades como prevenção, controle e tratamento da doença. Esta revisão integrativa objetivou identificar quais são as ações de enfermagem frente ao tratamento de pacientes com hanseníase. A revisão integrativa da literatura foi realizada a partir do levantamento bibliográfico de artigos científicos na base de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Base de Dados de Enfermagem (BDENF) e Scientific Electronic Library (Scielo). Como critérios de inclusão, consideram-se os artigos científicos originais e completos publicados em periódicos nacionais e internacionais em língua inglesa e portuguesa nos últimos dez anos. Foram encontrados 52 artigos, porém 43 artigos foram excluídos por não contemplar os critérios de inclusão da pesquisa, restando 9 artigos para análise do conteúdo, sendo 2 artigos da LILACS, 3 SCIELO e 4 BDENF. Uma das funções da enfermagem é capacitar a equipe, fazer avaliação dermatoneurológica, garantir universalidade e acessibilidade da assistência para todos os pacientes com hanseníase, realizar escuta e comunicação terapêuticas, estabelecer vínculo, confiança e compromisso com o enfermo, contribuindo para diminuir o índice de abandono do tratamento. O enfermeiro deve considerar a singularidade e subjetividade de cada paciente, oferecer apoio e prestar todo esclarecimento acerca da doença, orientar quanto ao uso adequado da medicação e quanto à prevenção de incapacidades, orientar práticas de autocuidado e desconforto decorrente do tratamento. Minimizar o estigma social que a doença ainda carrega, reduzindo o preconceito e a exclusão social dos doentes por meio de ações educativas para a comunidade, além de ser responsável pela supervisão do trabalho dos Agentes Comunitários de Saúde que orientam a procura da unidade de saúde em caso de suspeita de hanseníase. Nenhum dos artigos trouxe como a enfermagem atua no tratamento de pacientes em caso mais avançados de hanseníase com presença de incapacidades. BDENF foi a base de dados com mais artigos encontrados relacionados à temática. Após realização do estudo ficou evidente a importância da assistência de enfermagem ao paciente com hanseníase e suas atribuições.
